SUNY Potsdam Athletics

SUNY Potsdam student-athletes (SA's) must comply with all NCAA, SUNYAC, and SUNY Potsdam eligibility rules and regulations pertaining to meeting and maintaining athletic eligibility. Any prospective SA must complete the following steps before being cleared to participate in their first practice:

1) Sports Medicine Compliance Forms must be completed by the due date

2) Complete all mandatory NCAA compliance forms (available here online in early August)

NCAA regulations, required forms, and instructions for completion are available below. These forms must be completed online before attending the mandatory NCAA compliance meeting.

3) Attend a mandatory NCAA compliance meeting:

These meetings are conducted by the athletic department's compliance officer. Pertinent NCAA bylaws, campus academic standards for eligibility, and compliance issues will be covered at this meeting. SA's will also have the opportunity to ask questions and receive clarification about NCAA bylaws and other issues regarding eligibility.

1) 2017-18 SUMMARY OF NCAA REGULATIONS
                DO NOT PRINT THIS FORM. Read the information before completing any of the forms below.
                We will review this information at your team's compliance meeting.

2) 2017-18 NCAA BANNED DRUGS
                DO NOT PRINT THIS FORM. Read the information before signing the Drug Testing Consent form in #4 below.
                A copy will be provided for you at your team’s compliance meeting.

3) 2017-18 SUNY POTSDAM STUDENT-ATHLETE HISTORICAL QUESTIONNAIRE
                Complete form online. This must be completed before your team compliance meeting.

4) 2017-18 NCAA COMBINED COMPLIANCE FORMS
        These 3 forms have been combined into one online form for 2017-18.
        Student-Athlete Statement (Form 17-3c)
        NCAA Division III Drug Testing Consent  (Form 17-3f)
        Authorization/Consent for Disclosure of Protected Health Information to the NCAA

        Complete form online. This must be completed before your team compliance meeting.

5) GENERAL AMATEURISM AND ELIGIBILITY FORM FOR INTERNATIONAL AND SELECT STUDENT-ATHLETES (Form 17-10c)
                For international student-athletes competing for the first time in intercollegiate athletics.
                Complete this interactive form online, print, and then manually sign and date the form; bring to compliance meeting.
                Returning international students do not need to fill out this form.

                Complete and submit this form entirely online. This information is used for your online bio page.
