The Sports Medicine Staff is dedicated to serving the student-athletes of all 15 intercollegiate sports teams at the State University of New York at Potsdam. In order to best serve you, we have included below, directions for successful completion of all required Athletic Training Online/Paper Forms.
To participate in athletics at SUNY Potsdam you are required to complete medical forms specifically for the athletic training office. We subscribe to the Athletic Trainer System® (ATS) which is an electronic medical records system. It allows us to gather pertinent medical information from you the athlete, including your insurance provider, emergency contacts, medical history and contact information. We also track all your injuries and medical treatments via the ATS and therefore a history of your medical records are always accessible to us.
Please follow the directions below and use the images as a guide to help you navigate the system. If you are a returning athlete, please note that some layouts may have changed from the previous year. Below you will find directions for accessing the system as a
FRESHMEN/TRANSFER or
RETURNING athlete.
PHYSICALS

Only FRESHMEN/TRANSFER athletes must complete a physical.

It is to be completed by a physician on the form provided on ATS.

This form can be located in the ELECTRONIC DOCUMENTS section of the MAIN MENU (See image below).

Our office does NOT need immunization records.

Our office does NOT accept the optional SUNY Potsdam General Student Physical Form – that is to be returned to Student Health Services.

INTERNATIONAL STUDENT-ATHLETES

MUST obtain medical insurance that will cover costs associated with injury occurring during Collegiate Athletics participation in the United States.

The student insurance offered by the SUNY Potsdam International Education & Programs office does NOT cover athletics participation.

ADD/ADHD DIAGNOSIS

If a student-athlete is diagnosed with Attention Deficit Disorder (ADD) or Attention Deficit Hyperactivity Disorder (ADHD), AND taking prescription medication for control of this disorder, there must be a document on file from a physician confirming the diagnosis.

This must be on file prior to any participation in intercollegiate athletics.

Failure to document diagnosis and prescription medication(s) for ADHD may jeopardize a student-athlete's eligibility by ingesting a substance banned by the NCAA as per bylaw 31.2.3.
