SUNY Potsdam Athletics

The Sports Medicine Staff is dedicated to serving the student-athletes of all 15 intercollegiate sports teams at the State University of New York at Potsdam. In order to best serve you, we have included below, directions for successful completion of all required Athletic Training Online/Paper Forms.

To participate in athletics at SUNY Potsdam you are required to complete medical forms specifically for the athletic training office. We subscribe to the Athletic Trainer System® (ATS) which is an electronic medical records system. It allows us to gather pertinent medical information from you the athlete, including your insurance provider, emergency contacts, medical history and contact information. We also track all your injuries and medical treatments via the ATS and therefore a history of your medical records are always accessible to us.
 
Please follow the directions below and use the images as a guide to help you navigate the system.  If you are a returning athlete, please note that some layouts may have changed from the previous year.  Below you will find directions for accessing the system as a FRESHMEN/TRANSFER or RETURNING athlete.
 
PHYSICALS

• Only FRESHMEN/TRANSFER athletes must complete a physical.
•  It is to be completed by a physician on the form provided on ATS. 
• This form can be located in the ELECTRONIC DOCUMENTS section of the MAIN MENU (See image below).
• Our office does NOT need immunization records. 
• Our office does NOT accept the optional SUNY Potsdam General Student Physical Form – that is to be returned to Student Health Services.
• It can be handed into our office before participation or mailed to:

INTERNATIONAL STUDENT-ATHLETES

• MUST obtain medical insurance that will cover costs associated with injury occurring during Collegiate Athletics participation in the United States.
• The student insurance offered by the SUNY Potsdam International Education & Programs office does NOT cover athletics participation.
• Please contact Lisa Brown (brownli@potsdam.edu) for information regarding potential travelers insurance carriers that meet the required criteria.

ADD/ADHD DIAGNOSIS

• If a student-athlete is diagnosed with Attention Deficit Disorder (ADD) or Attention Deficit Hyperactivity Disorder (ADHD), AND taking prescription medication for control of this disorder, there must be a document on file from a physician confirming the diagnosis.
• The summary of required information can be located in the ELECTRONIC DOCUMENTS section of the MAIN MENU (See image below).
• This must be on file prior to any participation in intercollegiate athletics.
• Failure to document diagnosis and prescription medication(s) for ADHD may jeopardize a student-athlete's eligibility by ingesting a substance banned by the NCAA as per bylaw 31.2.3.

ATS DIRECTIONS

FRESHMEN/TRANSFER ATHLETES: Web Portal Link: potsdam2.atsusers.com To be used for first time login ONLY – Username: NEW Password: NEW Database Name: ATSPotsdam Once you login, you will see the GENERAL INFORMATION screen. You will create a secure username/Athlete ID and password for yourself. This will keep your information confidential. DO NOT SHARE IT.   Click SAVE, and new tabs will appear at the top of the screen. Please fill out each tab fully!   Click on MAIN MENU to access the ELECTRONIC DOCUMENTS – this contains: Physical Form ADD/ADHD Form Secondary Insurance Policy Information Concussion Information	RETURNING ATHLETES: Web Portal Link: potsdam2.atsusers.com To login – use Athlete ID and password you created for yourself the previous year. Do NOT create a new profile If your forgot your username – e-mail brownli@potsdam.edu   Update ANY & ALL tabs, including: Demographics – address, phone number, etc. Medications Health History Surgeries Changes in Insurance Changes in Emergency Contact   Must complete the PARTICIPATION STATEMENT under ATHLETE FORMS
